Hi again!  Here are the updated master lists of fall 2009.  Make sure your information is correct.  Contact us if we need to change anything.

Please note that the semesters marked are for the grade report used to qualify for a semester. For example, everyone this semester is marked for second semester, 2009, because everyone turned in a report card from that time period to qualify for this semester.

CSF Lifetime Member Status

Please note that you must qualify for 4 semesters to become a lifetime member. Those 4 semesters must be from the second semester of your sophomore year, either/both semesters of your junior year, and either/both semesters of your senior year.

Hi everyone!  As you may know, CSF is a club that emphasizes community service and high standards of scholarship.  YOU MUST REAPPLY FOR CSF FOR EVERY SEMESTER.  To get Lifetime Membership, high school students must qualify for CSF for four or more semesters.  Please follow the directions below to fill out the CSF form.  The CSF form is provided below for your convenience.  Bring the form in on Tuesday, September 15.  We will be holding a meeting in the upper gym at lunch.  See you there!

~PLEASE NOTE: Freshmen cannot apply first semester.  However, they can apply for CSF membership starting second semester.

 

Instructions on How to Complete the CSF Application 

 

On the short line at the top of application next to the words “ Fall Semester Membership 

Application, please write the year in which you are applying (e.g. 2009). 

 

Please attach the two-dollar membership fee to the application in cash (no checks). Do not 

directly staple the cash to the application and do not use a paper clip to attach the cash 

(suggestion- seal the cash in an envelope with your name written on it, and staple the top of the 

envelope to the application).  

 

Please provide a copy of your report card/progress report from the second semester of the 

2008-2009 school year. Please provide the semester report card, as quarter report cards will not 

be accepted (the ending date of the time period must say June 2009). If you are unable to locate 

your report card, you may provide your transcript that includes the second semester of last year 

on it (the ending date of the time period must say June 2009). Please staple the report 

card/transcript to the application as well. 

 

Please verify the number of semesters you have been in CSF with the master lists (Class of 2010 Master List, Class of 2011 Master List, Class of 2012 Master List). The columns 

marked represent the semester for which you turned in your report card, not the semester you 

were in CSF. For example, any person in the class of 2010 with a “1” marked in the column “F-06 

Frosh” (2006 freshman year, first semester) turned in a report card for that semester in order to 

be in CSF second semester of freshman year in 2007.

Please be aware that this year, it is mandatory for every member to complete 10 hours of community service per semester.  Please provide proof of community service at the end of each semester by getting signatures of those responsible for the activity.  Please fill out the CSF Community Service Hours Sheet, and turn it in at the end of the semester. (date and location to be announced)
